Purpose
Why the journal exists
Respiratory disease is studied in several places at once — at the bedside, in the physiology laboratory, across populations, and in the analysis of routinely collected data — and the findings that change practice are often the ones that travel between them. The journal exists to give that work one openly accessible scholarly record.
It publishes research that advances how respiratory disease is understood, measured, treated and prevented, and makes each accepted article free to read on publication, so clinicians, researchers and public-health readers can use it immediately.

The scope follows the full range the journal's title describes, across pediatric and adult respiratory medicine, and submissions are welcome across all of it. Published articles illustrate that range. The journal considers original research, reviews including systematic reviews, methods and protocols, short communications, and case reports.

These are the standards the journal holds itself to, and the terms on which it publishes.

How review is conducted
All submissions undergo initial editorial screening. Manuscripts that meet the journal's scope and minimum requirements proceed to independent peer review. Single-blind by default; double-blind review is available on request. Manuscripts that proceed to external peer review are normally evaluated by at least two independent subject-matter experts.

How decisions are reached
Editorial decisions are based on scope, scientific quality, methodological rigor, ethical compliance, reporting quality, and relevance to the journal.

Editorial independence
Those decisions remain independent of fees, services, membership, and editorial or reviewer roles. Editors recuse themselves from decisions on their own submissions, and those manuscripts are handled independently.

Authors are accountable for the integrity of the work they report, for disclosing funding and competing interests, and for holding the ethical approvals their study design requires. The editorial policies and author instructions set out these requirements in full.

Rights and reuse
Articles are published open access under a Creative Commons Attribution (CC BY 4.0) license. Authors retain copyright, and readers may reuse the work with attribution.
